At the Affirm, we believe healthcare and social service providers play a key role in meeting the needs of adolescents and empowering them to establish lifelong health behaviors. As a result, we launched The Arizona Alliance for Adolescent Health (AzAAH) and began implementing the Adolescent Champion Model (ACM) with the goal of improving health care delivery for adolescents. Additionally, AFHP runs the knowledge-sharing platform, The Collective Resource Hub, aimed to build the capacity of those who work with youth and connect like-minded partners.
